What is an apicoectomy

An apicoectomy is a microsurgical solution for situations where conventional root canal treatment has not eliminated the source of inflammation. Removing 2–3 mm of the infected root tip along with the cyst resolves the problem and saves the tooth.

When an apicoectomy is indicated:

failed prior root canal treatment
anatomical impediments to root canal treatment
cyst or granuloma unresponsive to conventional treatment
necessity of preserving a tooth with a crown or post

How the procedure is performed:

local anesthesia and gum incision
creation of a bone window to access the root apex
resection of 2–3 mm of the apex along with the cyst
retrograde root filling
suturing the wound and a follow-up X-ray

Prognosis after apicoectomy

When performed with proper indications and high quality, an apicoectomy ensures tooth preservation for 10 years or more. A follow-up X-ray after 6–12 months confirms bone regeneration in the surgical area. A stable radiographic image after one year is a criterion for a successful outcome.
