Surgical periodontology is applied when conservative treatment fails to reach the bottom of the pocket. The resective approach removes affected tissues and creates an anatomy conducive to home care. The regenerative approach restores what has been lost, utilizing biological materials and membranes.
The choice depends on the shape of the bone defect, as determined by CT scan. Horizontal bone resorption is an indication for the resective technique. Vertical defects with a three-dimensional bone pocket are an indication for the regenerative technique, where conditions for tissue regeneration exist. Often, both approaches are combined within a single procedure.
